I read that if the upper and lower agitators don't work together correctly, they can bind on clothes somehow and wear a hole. My upper agitator may not have been moving correctly (turn, stop, turn same way, while lower agitator goes turn, turn back, turn). It was very easy to remove with a socket wrench. It was somewhat worn away on the bottom, which I think could cause the problem. I replaced it and so far, all is well.
