At first I tried to fix the bent rails and press back in the ball bearings, but even the sleeves were ripped. The rails purchased from Appliance Part Pros were in-stock and lowest cost. I needed to keep the metal rod that keeps the two drawer slides in sync, and a small plastic piece that holds the metal rod in place.
