The rack roller had worn away on one side and would slip off the rail. Replacing the roller corrected the problem. I ordered an extra just in case one of the other rollers failed.
Just use a flat screwdriver to push the clip open enough to take the part off and snap the new one on. You'll be done in less than a minute.
