To diagnose, I tipped the washer back on its own' legs and pored a single cup of water into the drum. Water immediately poured out the bottom of the tub cover (big white container around the drum). I could see a quarter coin protruding through the hole. Apparently a quarter got between the drum and the cover and was hammered through the cover by a bolt head on the spinning drum.
