I found several other parts that needed to be replaced as well. Once All the parts were received, I again watched the video. The problem was most likely the evaporator fan motor but it could have been a bad blade, loose parts or bad motor mounts. Hard to tell without taking everything apart and inspecting. The fresh food door hinge lever assembly is a linkage mechanism at the fresh food section's door hinge that guides smooth door movement and helps maintain proper door alignment. It works in combination with the door cam to ensure the door closes evenly and seals tightly against the cabinet.
