At first I tried to fix the bent rails and press back in the ball bearings, but even the sleeves were ripped. The rails purchased from Appliance Part Pros were in-stock and lowest cost. I needed to keep the metal rod that keeps the two drawer slides in sync, and a small plastic piece that holds the metal rod in place. Ordered replacement clips. The replacements don't come with screws or the caps to cover the screws, so save all the hardware aside from the plastic hinges. You need to order the upper and lower halves of the hinges. The repair was very quick and easy, took about 5 minutes to attach the hinges to the shelf, and screw the shelf back into the side of the ice maker. Very happy to have the shelf back.

The left side sync wheel was broken so the right side wouldn't stay all of the way closed. Replaced the left side sliding rail assembly and installed a new right side gear. Repair took 25 minutes, most of that time was unloading and reloading the drawer w\ food.
