You need to order the upper and lower halves of the hinges. The repair was very quick and easy, took about 5 minutes to attach the hinges to the shelf, and screw the shelf back into the side of the ice maker. Very happy to have the shelf back.

Removed the vegetable drawers, removed the shelf above them. Removed two screws (philips #2) holding the left drawer rail in place. Levers the front of the rail away from the side of the refrigerator and pulled, sliding the tab at the rear of the rail out from the fridge wall.
Placing the new rail had some challenges due to extra plastic at the far end of the rail. I needed to carefully cut/trim (with large diagonal cutters) the plastic tab to match the old rail otherwise the extra plastic...
