At first I tried to fix the bent rails and press back in the ball bearings, but even the sleeves were ripped. The rails purchased from Appliance Part Pros were in-stock and lowest cost. I needed to keep the metal rod that keeps the two drawer slides in sync, and a small plastic piece that holds the metal rod in place. The rails slide out easily if you push a small plastic latch towards the back of the freezer section. I needed to back out 4 10mm bolts that hold the door to the rails. Just be careful not to tighten them too much they seemed like they would strip easily.
