Eventually determined that temperature in ice maker compartment was well below freezing but above the temp that the ice maker would trigger properly (about 20 deg F.). Replaced both the blower motor and air damper in the refrigerator/ice compartment which resolved the issue.. After bench testing determined that the damper worked fine but the blower motor had burned out.
