I replaced the hinges and hinge mounts. Upon inspection of the mounts, it seems that they had become slightly deformed on the lower portion, where the heel of the hinge applies pressure to the frame. Replacing the trim takes a little bit of alignment and light force. The trim slides over the glass panel of the door, with the glass sliding into plastic tabs. We kept the door nearly vertical (almost closed), lined up the tabs on the trim with the glass door, and used mild force to slide the trim down onto the glass door.
