Michael, With the bearing having fallen apart it would have most likely been caused by dirt and water getting into it. The inner tub bearing for this unit will be part number MAP61913708. The metal piece that was on the shaft would be part of the original seal and is not needed.

John, If you set the inner tub on end and kind of wiggle the outer tub to get it to drop down onto the shaft. Also make sure that you put just a thin layer of grease around the shaft to help it slide into place.
